Many patients are unable to give a reliable answer during their vision test. This may be from disability, young age, confusion, lack of language comprehension... or other causes. Further, automated refractors (computers) are often not accurate with some patients, especially infants, and others simply are unable to physically use standard testing equipment. This is when using this highly skill-dependent procedure really shines. Few doctors ever learn this art with any confidence, and fewer still use this during their eye exams. This is why so many doctors rely on using dilating drops on their younger patients, it is the only way an automatic refractor's results would be close to being accurate. Being well trained in this technique allows me to prescribe with confidence without using dilating drops in most cases. I still use dilating drops as indicated, but do to the high anxiety of our younger patients, my skills in retinoscopy often allow me to postpone dilation until they are more mature & understanding.
I have invested in the latest and most precise Streak retinoscope from the German company Heine, a tool which allows exact measurement of refractive errors such as myopia, hyperopia & complex astigmatism. Further, I also use the far less common Spot retinoscope- a device that is only found in the hands of highly specialized vision therapists & promoters of developmental vision care. Each version gives similar but unique information, and I will often use both on each "specialty" or child exam. I know of no other of my peers that do such. Used together I have full confidence in my prescribing ability... without causing undo discomfort in our children.

DEFINED: Retinoscopy is a technique to obtain an objective measurement of the prescription or refractive error of a patient's eyes. The examiner uses a retinoscope to shine light into the patient's eye and observes the reflex off the patient's retina. While moving the streak or spot of light across the pupil the examiner observes the relative movement of the reflex or manually places lenses over the eye (using trial lenses) to "neutralize" the reflex.
